ELIGIBILITY
Eligibility:
All full-time staff, at least 30 hours/wk, who are W2 employees are eligible to enroll in our health, dental, vision, life, and LTD insurance policies.
Launched Staff are required to be enrolled in our group health insurance plan beginning in the month they launch. Families of domestic staff are welcome to be enrolled in our plan but are not required to be if they have comparable coverage elsewhere. International families are required to be on our group health insurance plan unless they qualify for one of the exceptions listed below.
Exceptions
If domestic staff is covered by a spouse's group plan, the employee may opt to not be in the CMM plan. Being on a parent's plan is not an exception.
If an international staff's family is covered under another country's national health plan, that family is exempted from being enrolled in CMM's group plan. The employee, however, is still required to be enrolled in the plan.
Changes to Coverage:
Health Insurance - Changes to coverage can only be made during the open enrollment period or after a qualifying life event.
Dental & Vision Insurance - Changes to coverage can only be made during the open enrollment period of after a qualifying life event. Staff can also choose to add or drop coverage when changing from a domestic to an international assignment or visa versa.
Employer Paid / Staff Ministry Account Funded:
The CMM group health insurance plan is considered to be a 100% employer-paid plan. Insurance premiums are paid in full by the CMM and are not taxable benefits. The cost of the monthly insurance premium will be debited from the staff person's ministry account fund.

Our group health insurance plan is through United Health Care. Our plan has a single deductible option of $2,500. Any premiums are not allowed to be reimbursed per IRS regulations.
All covered staff has access to their own member portal at www.UHC.com. If you have never visited this site before, you will be required to register before logging in.

LIFE & AD&D INSURANCE
All CMM staff will also be enrolled in a $15,000 life insurance policy and a $15,000 basic AD&D policy through UHC, which provides a $15,000 benefit for the employee and spouse and a $5,000 benefit for children.
DENTAL
Employees eligible for CMM group health insurance will also be given the option of a CMM dental insurance plan managed by Delta Dental. Enrollment must be done either within 2 months of becoming eligible, after a life-change event, or during the annual open enrollment period in October. Monthly premiums are deducted from the staff person’s ministry account as a tax-free benefit. Delta Dental is not meant to be an international plan, so international staff is advised to not utilize this insurance unless they anticipate using the insurance during trips to the USA.

Enrollment must be done either within 2 months of becoming eligible, after a life-change event, or during the annual open enrollment period in October. Monthly premiums are deducted from the staff person’s ministry account as a tax-free benefit. Superior Vision is not meant to be an international plan, so international staff is advised to not utilize this insurance unless they anticipate using the insurance during trips to the USA or want their prescription contacts and glasses shipped.

LONG-TERM DISABILITY
The CMM has a long-term disability group plan for all US and international full-time employees (not spouses). Employees will be enrolled automatically upon their initial enrollment in medical insurance.
The cost per employee is $15 per month, which is a tax-free benefit and will be deducted monthly from your ministry account.

INTERNATIONAL LIABILITY
The CMM carries a general liability insurance policy for staff and volunteers traveling outside of the USA. This covers emergency medical evacuation, repatriation, and other liability coverage in case of accidents or injury to third parties (such as a car wreck).

DEPENDENT CARE
A Dependent Care Flexible Spending Account, or “DC FSA,” is a pre-tax benefit account used to pay for dependent care services while you are at work. The money you contribute to a DC FSA is not subject to payroll taxes, so you end up paying less in taxes and taking home more of your paycheck. Under this type of account, a “dependent “ is a child under 13 years of age (until the day of their 13th birthday) and adult dependent who can't take care of themselves. Please keep in mind that they must live with you and be claimed as dependents on your tax return. The monthly contribution will be deducted from your payroll and contributed to your DC-FSA account administered by WageWorks.
The IRS allows employees to contribute $5,000 per year to this type of FSA. Once these contributions are made, you will see these funds appear in your WageWorks employee portal. These funds can be used to pay for eligible dependent care expenses. You can access these funds via reimbursement or by requesting WageWorks to pay your childcare provider directly. You can find more information about this here.
Staff must enroll in this benefit during the annual open enrollment period. At that time, staff must elect the total amount they wish to contribute for the following year. This amount will be evenly spread against all 12 pay periods in the year. For instance, if you chose the maximum, 5,000, then you will have 416.67 deducted from each monthly payroll and contributed to your DC FSA account at WageWorks. Furthermore, unless you have a qualifying life event, you are not permitted to change your election until next years open-enrollment period.
Unfortunately, this benefit does not apply to primary or secondary education, so utilizing it to pay for international school is not an option.

REIMBURSEMENT ARRANGEMENT
A Health Reimbursement Arrangement (HRA) is a pre-tax benefit set up by an employer through which employees are allowed to reimburse eligible medical expenses for themselves and their dependents. CMM has an HRA that all support-raised staff who are enrolled in our group health insurance plan are eligible to participate in. By design, an HRA is an employer-funded benefit. However, as this benefit is only available to support raised staff, each staff is responsible for raising funds related to their use of CMM's HRA.
WageWorks administers CMM's HRA on our behalf. All eligible staff will be enrolled in our HRA and will receive an automated email from WageWorks with prompts to register their online account. Staff can find useful information, track their claims and request reimbursements through this online account. WageWorks also has a mobile app, EZ Receipts, that may be used to remotely record medical expenses for reimbursement.
Staff are eligible to reimburse up to the below limits depending on their enrollment status in our group health insurance plan.
Family, Couple, Single w/children - 13,000
Singles - 6,500
Using HRA Funds:
WageWorks has three standard ways in which staff can utilize their HRA.
"Pay By Card" - All staff enrolled in the HRA will be issued a debit card. This card can be used for eligible medical expenses as listed in your WageWorks employee portal. Staff living in the US will have their card shipped to their home. Staff living outside the US will have their card shipped to the home office.Charges from the card will be automatically synced to your online account. It is important that you upload receipts from each transaction.
"Pay Me Back" - Staff can pay for qualified medical expenses out of pocket and submit the expense for reimbursement via their online employee portal or the EZ Receipts mobile application.
"Pay My Provider" - Wageworks has an online billpay feature. Staff can request Wageworks pay provider directly for qualified medical expenses.
Per IRS regulation, receipts are required for ALL EXPENSES.
Funds utilized through the CMM HRA will be deducted from your ministry account.
Registering Your WageWorks Account:
Once you are enrolled in our WageWorks HRA, you should receive an automated welcome email from WageWorks. Once you receive this email, you need to visit WageWorks.com and register your online account. On the home page, select "Employee Registration" from the "Login/Register" drop down list. This will open a new page. You will be prompted to provide some information to register your account. This information will vary slightly depending on whether you are living in the US or abroad.
In the US - You will use your own zip code to register your account.
Outside the US - You will use the CMM's PO Box zip code, 72702, to register your account.
After you register your online account, you should download the EZ Receipts app and use the login credentials you created when registering your online account.
